Insurance
We participate in: Anthem, Indiana Medicaid, SIHO, Encore, Sagamore, PHCS, IHN, Aetna, Cigna, United Healthcare and Humana. A current card must be presented at every visit in order for us to file your claim. Co-pays and deductibles are due at time of service.
You must also present your insurance card for non-contracted insurances that you wish us to file on your behalf. Payment responsibility remains yours. If your insurance does not satisfy the claim within 30 days, the balance is then due from you.
